Advanced Mail - HELP DESPERATELY NEEDED

PLEASE HELP. I am a complete novice at sending forms via email.

I've purchased Advanced Mail.
1.unable to download demo tutorial.
The following error occurs:

the file'c:\DOCUM~\Martina\LOCALS~1\Temp\AdvancedMailDemo-mxi\sqlWriteSQL.htm'is missing.

2.I am using Dreamweaver 8. I have taken off the Advanced Mail extension to download the demo tutorial.

3. I have a form on my website for clients to complete.I have tried following the flash tutorial.
I have set up the server info. - no problem.
In the mail section i am unsure of what to put in the To box. I would like the info sent to myself and a copy returned to the client.
3. Unsure whether I have to put form in body.
4. Unsure of what the replace tag is for.
5. Pressed ok but I keep getting the following error:

JavaScript error while loading CDONewMail.html:Unable to open script file"C:\Program Files\Macromedia\Dreamweaver8\Configuration\ServerBehavious\Shared\MM\Scripts\CMN\UI.js"(error3)
