Update Record Upload - not deleting existing file

I've got the update record upload working fine, but it does not delete the existing file, despite the fact that I've select "yes" and told it what the db field name is. I'm not getting any errors. It's just not removing the existing file. Instead it updates the db, and uploads the new file. In theory I could live with this but I like to keep the upload directory clean with no orphaned files. Let me know if I'm missing something.
