Date Picker Button or image

New to the forum - when I'm adding the date picker to my form I'm able to fill in everything except the "Select button or image" d_r_o_pdown. It does not even give me the option to choose anything; would really appreciate any help.

I think you need to put a user document together for the Forms controls - I know there's a write up about what the fields are but that does not give you the details how you set the module up, this would be very helpful.
