Here's also a little more info:

I tried running the same extension on another site I have on an entirely different web server, which also only has ASP Jpeg and it will not create thumbnails on that site either. All other aspects of the extension seem to work ok.

Additionally, this second site that I tried it on also has .NET 1.1 installed too and apparently the extension could not use that in auto detect mode. I also tried specifically selecting ASP Jpeg for server component (instead of auto detect). No luck with that either.
