I figured it out - it will not create a thumbnail at all (reglardless of server component) UNLESS you resize the main image. I previously did not want to resize the main image being uploaded at all and was only using the second option to create a thumbnail.

I started testing all different ways, and once I selected the resize option, it finally created the thumbnail. Why do you have to resize to get a thumbnail created?

Anyway, I hope this solution helps someone else since I wasted my morning figuring this out. Seems like a bug in the extension to me. Any chance of getting a new version of the extension that does not require using resize in order to get a thumbnail created?
