file uploaded bu NO data written to db

Hi,
I recently purchased Ajax upload extension and followed the flas tutorial instructions.
The file is uploaded to the folder i choose on the uploade page of the extension, but no entry is written to the database.

The setup is the following: Acces DB 2007, with one filed called File1 as attachment, and some other fiels for the username, date of upload etc.
After chosing a file to upload (pdf-file) the progress bar comes up, confirms the upload, redirects me to the page I want, but NO data was written to the db, even no error came up. But, the file is stored in the upload folder on the server.

Any help how to resolve this? Thank you.
